摘要 對輕、中度痔應用電腦治療儀與傳統手術治療效果進行對比分析，期望為痔手術方案的選擇提供依據。將需手術治療的輕中度痔患者 100 例，隨機分為兩組各 50 例，分別應用電腦治療儀與傳統手術治療。觀察術后疼痛、排尿、出血情況，基本恢復一般正常工作時間及隨訪 1 年的療效。兩組手術時間、術后疼痛、排尿情況均無顯著差別。術后無大出血。兩組病例隨訪 1 年，無肛門狹窄、肛門功能障礙。結果表明，對輕、中度痔的手術治療，兩組手術方法都具有安全、有效、簡便、省時的特點，但電腦綜合治療儀能縮短治療時間。

Comparison of the Therapeutic Efficacy of Computerized Inserument Therapy

Vs Traditional Surgical Therapy for Mild and Moderate Hemorrhoids

Abstract The therapeutic efficacy of compyterized instrument therapy and traditional surgical therapy for mild and moderate hemorrhoids was compared in an attempt of providing basis for therapeutic choice . One hundred patients with mild or moderate hemorrhoids were divided randomly into two groups , 50 for each group ,receiving computerized instrument therapy and traditional surgical therapy respectively . The aspects of postoperative pain , frequency of urination , bleeding , recovery status and findings of one-year follow up in the two groups were compared . As results , there were no differences in operation duration , postoperative pain and urination between the two groups . There was no postoperative hematorrhea and no anal stricture or anal malfunction in both groups follow-up one year . It is concluded that these two therapies are all safe , is better in terms of shortering treatment duration of time .

應用電腦治療儀治療痔 50 例，并與傳統手術治療的 50 例進行療效對比觀察，現將結果報告如下。

•  資料與方法

1.1 臨床資料 需手術治療的輕、中度痔患者 100 例，發病部門占肛門 1/2 周徑以內。隨機分為兩組各 50 例，分別應用電腦治療儀（電腦組）與傳統手術（傳統組）治療。電腦組 50 例，男 24 例，女 26 例；年齡（ 45 ± 12 ）歲（ 21~73 歲）。

1.2 治療方法 術前清潔灌腸，肛周局部麻醉。電腦組：以電腦綜合治療儀切除痔核。傳統組：以外剝內扎和結扎注射方法處理痔核。術后均正常飲食，給予抗炎、輸液、換藥、對癥等治療。

1.3 觀察指標 手術時間、術后疼痛、排尿、出血、基本恢復一般正常工作時間，隨訪 1 年的療效。疼痛程度評估采用主訴疼痛分級（ visual analogue scale , VAS ）及數字評分法（ numeric rating scale , NRS ） [1] 將疼痛程度分為無痛、輕、中、重及極度疼痛。

2 結果

2.1 手術時間 電腦組：（ 10 ± 3 ） min （ 6~12min ） , 傳統組：（ 11 ± 4 ） min(6~24min)(p ＞ 0.05) 。

2.2 疼痛出現時間及強度 電腦組：手術后麻醉藥逐漸失效平均約（ 27 ± 8 ） min(0~51min) ，患者感動肛門疼痛。無痛、輕度疼痛 37 例（ 74% ），中度疼痛 12 例（ 24% ），重度疼痛 1 例（ 2% ）。傳統組：手術后麻醉藥逐漸失效平均（ 25 ± 9 ） min （ 0~52min ），患者感到肛門疼痛。無痛、輕度疼痛 36 例（ 72% ），中度疼痛 13 例（ 26% ），重度疼痛 1 例（ 2% ）。兩組無統計學意義。

2.3 排尿情況 術后部分患者因肛門括約肌痙攣疼痛出現排尿困難，但無導尿病例。平均導尿時間電腦組為（ 135 ± 36 ） mi n （ 35 ~ 360min ），傳統組為（ 121 ± 33 ） min(25 ~330min) 。兩組無統計學意義。

2.4 出血情況 兩組病例術中及術后均未發生大出血。

2.5 恢復工作時間 電腦組：（ 10 ± 4 ） d （ 2~15d ）；傳統組：（ 18 ± 3 ） d （ 8~20 ） d 。兩組有統計學意義（ p ＜ 0.01 ）。無術后感染者。

2.6 隨訪 兩組病例術后隨訪 1 年均無肛門狹窄、肛門功能障礙。兩組術后 3~11 個月，均有 3 例手術再次治療，占 6% 。

3 討論

對輕、中度痔應用電腦治療儀與傳統手術治療效果進行對比分析，期望為痔手術治療方案的選擇提供依據。結果顯示，對需手術治療的輕、中度痔，兩種方法都具有安全、有效、簡便、省時的特點。手術時間僅需 10 余分鐘，術后 70% 為輕度疼痛，排尿情況良好， 2~3h 內自主排尿，無需導尿。術中、術后無大出血，術后無創口感染。電腦綜合治療儀直接封閉血管，切除痔核，恢復時間縮短。兩組病例隨訪 1 年，無肛門狹窄和肛門功能障礙。
